The data is not stored publicly accessible and will be deleted after the application process is complete, unless statutory retention obligations require otherwise.

10. Online-Aufnahmebogen

Wenn Sie unseren Online-Aufnahmebogen ausfüllen, verarbeiten wir die von Ihnen angegebenen Daten – einschließlich Gesundheitsdaten nach Art. 9 DSGVO (z. B. Vorerkrankungen, Medikamente, Allergien) – ausschließlich zur Vorbereitung Ihrer Behandlung. Rechtsgrundlage ist Ihre ausdrückliche Einwilligung gemäß Art. 9 Abs. 2 lit. a DSGVO in Verbindung mit Art. 6 Abs. 1 lit. a DSGVO. Aus Ihren Angaben wird ein PDF-Dokument erstellt und per E-Mail an die Praxis übermittelt; eine Speicherung Ihrer Angaben auf der Website oder in einer Datenbank erfolgt nicht. Einen optional hochgeladenen Medikationsplan speichern wir nur vorübergehend bis zum Absenden des Bogens auf unserem Server, übermitteln ihn zusammen mit dem PDF an die Praxis und löschen ihn anschließend; Uploads aus nicht abgeschickten Bögen werden spätestens nach 24 Stunden automatisch gelöscht. Die E-Mail-Übertragung erfolgt verschlüsselt. Sie können Ihre Einwilligung jederzeit für die Zukunft widerrufen, indem Sie uns unter datenschutz@tuelay-onan.de kontaktieren.
